For the Shared-Ride Program, request an application from CCT Connect Customer Service at (215) 580-7145 or complete the online form and send it in along with proof of age.
ADA Paratransit Registration
ADA Paratransit Service applicants must call CCT Connect Customer Service to begin the registration process. A customer service representative will ask pre-screening questions and arrange to mail an application.
Once you return your completed application, you will be scheduled for a functional assessment.
The assessment evaluates an applicant’s physical or cognitive functional abilities. There is an interview about the applicant’s disability. Tests are administered by licensed professionals. The assessment should take no more than an hour, and CCT will provide free transportation to and from the assessment site.
Within 21 days of CCT receiving your application, you will receive an eligibility determination. If you are eligible, you will receive a new ID card.
